New Patient Information & Policies
First Appointment
For in-person sessions, please bring in paperwork or arrive 15 minutes prior to the first appointment in order to complete new patient paperwork.
For remote sessions, please complete the forms (below) and email to our office prior to your appointment.
Appointment Durations
- New Patient Evaluations: 75 minutes
- Follow-Up Sessions: 25 or 50 minutes
Dr. Goble will provide an initial diagnosis, discuss treatment options, and recommend follow-up appointments as clinically indicated.
Safety Measures
Please consider wearing a mask or changing appointment to a remote session if feeling ill or experiencing respiratory symptoms of coughing or sneezing.

To cancel or reschedule an appointment, please call the office at 480.661.3877.
Payment
Full payment is due at the time of scheduling.
Full refund is provided if cancelled 2 FULL BUSINESS DAYS prior to appointment. (Example: Monday 9 am appt must be cancelled by PREVIOUS Wednesday 9 am. Tuesday 9 am appt must be cancelled by PREVIOUS Thursday 9 am.)
Dr. Goble is an out-of-network provider and does not contract with any insurance carriers. Fees for service are due at the time of appointment and may be paid by cash, debit or credit card. Patients seeking to submit a claim with their insurance carrier for out-of-network reimbursement may request the necessary documentation to support the claim.
